FINANCIAL AID REQUEST FORM

SPHS Lady Boosters is now soliciting Financial Aid Requests for the new school year, 2007-2008.  If you are interested in obtaining assistance from SPHS Lady Boosters, please complete a Financial Aid Request (available for download below), and if appropriate, attach two written bids for items to be purchased. 

Financial Aid Requests are considered at each regular SPHS Lady Boosters meeting (not including our Holiday Dinner in December and our Installation Dinner in June).  In order for a Financial Aid Request to be considered, it must be turned in to our mail box in the SPHS Main Office on or before the first day of the month.  Our First Vice President will contact the requestor after the form and any attachments is received and invite the requestor to attend our next regular meeting to present the request in person to the membership.

Just above this paragraph is a small symbol that will link to the current Financial Aid Request form.  Please right click on the symbol; a box will appear; scroll down and click on 'Save Target As'.  A new box will open and you will be able to save the form to a file or folder on your computer, and then print it out for use. Or, you can pick up an application from Judy Davenport in the Main Office at SPHS if you prefer.

How Lady Boosters Considers & Administers Financial Aid Requests

Financial Aid is offered in several forms to all legitimate groups, clubs, classes, and athletic teams organized at San Pedro High School.  Most of the time, Financial Aid consists of the SPHS Lady Boosters making a cash contribution to the group, coach or teacher requesting aid. We also may choose to purchase the item(s) requested and donate them directly rather than providing cash or a check.  Occasionally, financial aid consists of Lady Boosters requesting assistance from local merchants to benefit students at SPHS are provided by those merchants in the form of food and/or beverages that are donated to SPHS students through Lady Boosters.

We do not fund financial aid requests for repairs, equipment or supplies that have already been provided to a teacher, club, organization or athletic team by a vendor or supplier, nor do we fund requests for food, snacks, beverages or awards banquets.

As a group, we prefer to make monetary contributions for items that will last for many years. Equipment of many kinds is one of our favorite things to finance as it usually benefits many students over many years. Over the past few years we have helped purchase video equipment for the football program, DVD/VCR combo players for foreighn language classrooms, overhead projector for use with special education math, reupholstery of weight room equipment, supplied a new telephone to the Leadership classroom, helped purchase new band uniforms , purchased two new band instruments, and we purchased a new state-of-the-art LED softball scoreboard honoring the years of Girls' Softball Teams who have brought L.A. City Championships to SPHS. 

Under the best conditions, we also prefer that a broad range of students benefit from the assistance, and that the group requesting assistance has demonstrated that they have fundraised for their activity prior to or while they are seeking aid from us. And we encourage any group approaching us for funds to also apply for aid to the SPHS Pirate Boosters organization so that both groups may be of assistance.

The Lady Boosters First Vice-President is charged with the responsibility of reviewing all Financial Aid Request forms, contacting the primary requestor (usually a faculty member), arranging for the requestor or his/her adult representative to attend our next regular meeting, and notifying the requestor of the disposition of the request.

Our requirements are simple:  Complete the form entirely.  A signature from the club, group, class or athletic team coach must appear on the form.  If the request is for equipment or supplies, please attach a minimum of three (3) written bids for the items to your completed Financial Aid Request form.

Submit the completed form to the SPHS Lady Booster mail box (in the Main Office) on or before the 1st of any month so that the request may be reviewed, and the requestor notified, prior to the next regular meeting scheduled. That is, if you have a request and submit it to us by November 1, 2006, you will be contacted prior to our meeing scheduled for the second Tuesday of November and invited to attend our meeting and present your Financial Aid Request to the membership.

Financial Aid Requests are not normally considered at our dinner meeting held to celebrate the Holidays in December of each year, or at the June Installation Dinner meeting. If your request is an emergency or urgent status, please attach a note to the form when you submit it so that we may make an exception.
